Planning to opt out of coverage? You can complete and submit your opt-out form online via DocuSign at https://tinyurl.com/tehwoptout

TEHW Hours of Operation: Monday–Friday, 8:30 AM–5:00 PM

We will also offer extended hours on the following dates during Open Enrollment:

• Tuesday, November 4, 2025: 5:00 PM–8:30 PM
• Saturday, November 8, 2025: 9:00 AM–12:30 PM

All forms must be submitted by Friday, November 14, 2025, at 5:00 PM.
